UNKNOWN BIOLOGICAL FATHER LAST KNOWN ADDRESS: UNKNOWN PRESENT WHEREABOUTS: UNKNOWN In accordance with O.C.G.A. 15-11-284 you are hereby notified that this proceeding and the hearing specified herein is for the purposes of terminating parental rights. You are hereby notified that Georgia law provides that you can permanently lose your rights as a parent. A petition to terminate parental rights has been filed by the Douglas County Department of Family and Children Services requesting the Court to terminate your parental rights to your children. A copy of the Petition can be obtained from the Clerk located at 8700 Hospital Drive, Douglasville, Georgia 30133. This is a summons requiring you to be in Court. If you fail to come to Court as required you may be held in contempt of Court and punished accordingly. The Court can also terminate your rights in your absence. Now therefore, you, the party named above are commanded to be and appear on the date and time stated below, and to remain in attendance from hour to hour, day to day, month to month and year to year as the case may be continued, and until discharged by the Court, and you are commanded to lay all other business aside and to be and appear before the Juvenile Court of Douglas County, Georgia located at 8700 Hospital Drive, Douglasville, Georgia 30133 on January 17, 2018 at 11:00 a.m. You are ordered to appear and make a defense thereto and to show cause why the child should not be dealt with according to the law. If the court at the trial finds that the facts set out in the petition to terminate your parental rights are true and that termination of your rights will serve the best interests of your child, the court can enter a judgment ending your rights to your child. A parent is not thereafter entitled to notice of proceedings for the adoption of the child by another nor has he or she any right to object to the adoption or otherwise to participate in the proceedings. If the judgment terminates your parental rights, you will no longer have any rights to your child or make any decisions affecting your child or your child’s earnings or property. Your child will be legally freed to be adopted by someone else. Even if your parental rights are terminated: (1) You will still be responsible for providing financial support (child support payments) for your child’s care unless and until your child is adopted; and (2) Your child can still inherit from you unless and until your child is adopted.

Pursuant to O.C.G.A. § 15-11-283, notice is given to you as the putative father that is not the legal father of the child, that unless you file a Petition to Legitimate said child pursuant to O.C.G.A. § 19-7-22 within 30 days of your receipt of this notice and file notice of such filing with this Court you have no right to object to the termination of your parental rights in said child and your parental rights will be summarily terminated. This is a very serious matter. The child and other parties involved may be represented by a lawyer at all stages of these proceedings. You should contact an attorney immediately so that you can be prepared for the court hearing. You have the right to hire an attorney and to have him or her represent you. If you cannot afford to hire an attorney, the court will appoint an attorney if the court finds that you are an indigent person. Whether or not you decide to hire an attorney, you have the right to attend the hearing of your case, to call witnesses on your behalf, and to question those witnesses brought against you.
