Dr. David Ray Hooks D.O., retired physician, age 67, of Talala, Oklahoma, passed away early Wednesday morning December 20, 2017. He was born August 29, 1950, in Brunswick, Georgia, to the late Walton Audily Hooks and Sallie Mae Tarte Hooks.
He was a veteran of the U.S. Navy who served on the USS Enterprise (CVAN65) during the Vietnam Conflict. Following his military service he was ordained as a minister of the gospel by the Assemblies of God and pastored churches in Georgia and Oklahoma. He continued in Christian ministry and in his devout love of Christ for the remainder of his life. He graduated from Oklahoma State University College of Osteopathic Medicine in 1990, and entered his medical career in family practice. He later served the communities of Bremen, Georgia, as a director of the Emergency Department, and as medical director of West Carroll Family Healthcare Center in Bowdon, Georgia, where he retired from medical service.
He is preceded in death by his parents, Audily and Sallie Hooks, and is survived by his wife of 42 years, Cheryl Jean Michael Hooks of Talala, Oklahome; his three sons and their families, Benjamin and Melisa Hooks of Talala, Oklahoma, Jonathan and Renee of Villa Rica, Georgia, and Thomas and Laura Hooks of Columbus, Ohio; his brother and his wife, Walton and Diane Hooks of Chesapeake, Virginia; his eight grandchildren, Zane Hooks, Trinity Hooks, Uriah Hooks, Caitriona Hooks, Sarah Hooks, Paul Hooks, Elias Hooks, Eviana Hooks.
Visitation will be December 26, 2017, from 9 a.m. to 5 p.m. and December 27, 2017, from 9 a.m. to 8 p.m. at Collinsville Dolton Funeral Home in Collinsville, Oklahoma.
Funeral services will be conducted Thursday, December 28, at 10 a.m. officiated by Pastor Kent Jensen at Oologah Assembly of God, 1346 U.S. Highway 169, Oologah, OK 74053.
Interment to follow at Oak Hill Cemetery in Oologah with military honors.
